Abstract: 

We describe a 70-year-old man who presented to our emergency department with worsening SHORTNESS of BREATH. His chest roentgenogram showed pulmonary congestion and his condition improved with heart failure treatment yet a repeat chest roentgenogram revealed an abnormality which had remained unnoticed in his first chest x-ray due to the underlying congestion in lung fields. Descending aorta was dilated and very prominent. Further work-up showed a type B dissection which seemed to be chronic. Considering the chronicity of the condition, the fact that it was actually asymptomatic and the patient’s fragile condition we decided not to proceed with surgery.

Abstract: 

BACKGROUND AND OBJECTIVE: Incidence of thymic lesions in the form of mediastinal cyst is very rare and is accompanied by risk of malignancy, which may induce signs of compression in mediastinum or accidentally be found during imaging. A case of mediastinal thymic cyst with compression-induced SHORTNESS of BREATH is presented.CASE REPORT: A 40 year old woman underwent diagnostic assessment because of SHORTNESS of BREATH. Considering imaging results regarding upper mediastinal cystic lesion with possible origin of thymus, she underwent excision of mass and middle sternum. Histopathologic examination reported a simple thymic cyst.CONCLUSION: Considering respiratory problems of patients with mediastinal cystic lesion, timely diagnosis and treatment is necessary to prevent probable malignancy in addition to relieving symptoms.

Abstract: 

Introduction: Pulmonary hypertension is common among patients with hyperthyroidism, and Graves’ disease constitutes the most common cause of thyrotoxicosis.Case Presentation: We report the case of a female patient admitted to the cardiology department with SHORTNESS of BREATH and pretibial myxedema. The diagnostic work-up revealed combined pre- and post-capillary pulmonary hypertension due to Graves’ disease superimposed on left ventricular diastolic dysfunction. Restoration of thyroid function led to normalization of the pulmonary pressure and symptom resolution.Conclusions: Thyroid disease is a cause of reversible pulmonary hypertension and thus should be appropriately considered in the diagnostic algorithm in patients with dyspnea, clinical signs of hyperthyroidism and elevated pulmonary pressure.

Abstract: 

Introduction: Gaucher Disease is an autosomal recessive lysosomal storage disease. Pulmonary involvement in Gaucher Disease is rare and often seen in the severe form of the disease with the worst outcome. Case Presentation: A 30-year-old man and known case of Gaucher Disease presented to our clinic with history of progressive dyspnea since 8 months ago. Pulmonary function test showed restrictive pattern. Chest CT scan revealed diffuse bilateral interlobular septal thickening and small interstitial nodules with ground glass opacities in lower lobes. Conclusion: Patients with Gaucher Disease that present with progressive dyspnea may have a manifestation of interstitial or alveolar lung disease.

Abstract: 

Background & aim: Fatigue and SHORTNESS of BREATH are the main complaints of patients after COVID-19 disease, and determining the factors associated with them is of high importance for effective care of these patients. The aim of this study was to investigate the relationship between fatigue severity and SHORTNESS of BREATH with arterial blood oxygen saturation (SPO2) in COVID-19 patients. Methods: The present study was a cross-sectional study. In order to carry the study, 193 patients with COVID-19 admitted to Fatemeh Al-Zahra and Lolagar hospitals in Tehran in the second half of 2021 were selected through convenience sampling method. Demographic information questionnaire, fatigue severity scale, Modified Borg Dyspnoea Scale and recording SPO2 form were used to collect data. Data were analyzed using SPSS26 statistical software using statistical tests. Results: In total, 56. 5% of the patients were women, 72% were married, 40. 9% were housewives and 82. 9% were over 40 years old. Patients' fatigue intensity (35 ±,17. 55) was low, arterial blood oxygen saturation (89. 74 ±,5. 35) and patients' SHORTNESS of BREATH (3. 35 ±,2. 03) were at the level of light activity. Based on the results, the relationship between fatigue intensity and oxygen saturation was not significant (P = 0. 246). There was a significant negative relationship between the severity of SHORTNESS of BREATH and SPO2 (P = 0. 005, r =-0. 202). As the severity of SHORTNESS of BREATH increased, the SPO2 decreased. Conclusion: The fatigue severity in patients with COVID-19 was moderate and the severity of SHORTNESS of BREATH was high, which should be given more attention as a traumatic complication. Also, due to the significant relationship between the severity of SHORTNESS of BREATH and arterial blood oxygen saturation, it is necessary for nurses caring for these patients to be reminded of the importance of measuring oxygen levels to seek the help of nurses and physicians in emergencies.

Abstract: 

Background: Anxiety is a special kind of anxiety that athletes competing in major competitions threshold to do so. The physical dimensions of anxiety symptoms such as tremor, palpitations, SHORTNESS of BREATH and symptoms such as cognitive symptoms include fear of failure and fear of poor performance, will occur. One of the psychological skills that are thought to be effective in reducing and controlling athletes' competitive anxiety, relaxation techniques are. It generally consists of two methods based on relaxation and cognitive. In this way, progressive muscle relaxation, has been used.Methods: This study consisted of 30 players, club Zob Ahan, randomly assigned to experimental and control groups. First of all players by anxiety questionnaires-Competitive CSAI-2 pre-test was performed. The players were in the experimental group received 20 sessions of relaxation, then step test was performed.Findings: The results were statistically analyzed using one-way ANOVA with post hoc test, the paired t test, t- test for independent groups, respectively (P=0.05), showed a progressive muscle relaxation rate reduce anxiety 's physical, cognitive anxiety brought down close to the border between the players resting heart rate is significantly reduced.Conclusion: The results showed that the goalkeepers and defenders, midfielders had significantly more cognitive anxiety but is not significant in comparison to the quarterback. have. And the native players one of the three subscales of cognitive anxiety, somatic anxiety and self-esteem were not significantly different.
